Admin Form page type: No page form #178

What steps will reproduce the problem?
1. Login to admin and create a new page
2. Choose page type "form"
3. Try to edit the page

What is the expected output? What do you see instead?
Expected:
To be able to edit the page.

Instead:
Blank page
