Problem with todonotes package

What steps will reproduce the problem?
Compile the attached tex file.

What is the expected output? What do you see instead?
Borders are supposed to be drawn around the notes in the margins and a line
is supposed to be drawn to the point in the text where the todo note
originates.

pdf of expected output is attached also.

What version of the product are you using? On what browser?
Preview release in Firefox (Ubuntu) and also tried in Chrome (Windows)

Please provide any additional information below.
todonotes is loads these packages

ifthen
tikz
calc
xkeyval
graphicx 
xcolor

I couldnt see any errors associated with these, but maybe a version conflict?

It appears that todonotes.sty isn't installed on LaTeX Lab.  It would be great 
if this style file was added.  Here is the package home:
