Email address autofill not working

Which Minimalist for Gmail option is causing the problem?
I don't know.

What is the problem? Describe completely:
A few days ago the autofill feature on the recipient fields stopped working.  
The suggestions still pop up when I start typing a name or address, but when I 
click on one or hit enter it doesn't populate the field.  Also, when I click on 
"add cc:" or "add bcc:" the link disappears but the email recipient field 
doesn't appear.  Behavior persists after reboot.  

Does the problem persist when loading Gmail with this link:

Yes, problem persists when loading with labs disabled.
