Summary:

This patch changes the behavior of RubyCAS-Client to request a PGT only
once per PGT IOU.

Holding off on this for now as it would not work with the current 
implementation of
the proxy callback controller bundled with the client. I may make this 
functionality
optional, so that it has to be explicitly enabled via a config setting 
(otherwise
PGT-IOU's are one use only by default).
