Automatically Enter Name into "Prepared by" #102

For a NEW work order, automatically add the string "[Name]: [Phone/Email]" in 
the "Prepared by:" field on the Intake Form.

For an EXISTING work order, automatically append the string, ", [Name]: 
[Phone/Email]" to the existing text in the "Prepared by:" field.

The name and Phone/Email is stored only for the session. See Issue 101.

Note: This requires completion of Issue 101, Part 3.2
