Create "Assigned To:" Field #107

Add the field "Assigned To:" under "Current Status:" on the Intake Form.  
However, the field should ONLY appear when the "Current Status" DOES NOT EQUAL 
"Open, unassigned".

If the status is currently "Open, unassigned", then the field should not appear 
on the form, and the value of the field should be <blank>.

If the status is changed from something TO "Open, unassigned", then the field 
should: 1. no longer appear on the form; 2. be cleared of data when the record 
is saved.

This field is free-form, and may be used for names, phone numbers, email 
addresses, etc. It is essentially to help organizations keep track of which 
team leader to whom a work order was assigned.
