"Assigned to" Appears on Info Box when Status changed to "Open, Assigned" #108

On the Info Box (on the map), the field "Assigned To:" should appear IF and 
ONLY IF the status is equal to or is changed to "Open, assigned."
Add the field "Assigned To:" under "Current Status:" on the Intake Form.  
However, the field should ONLY appear when the "Current Status" DOES NOT EQUAL 
"Open, unassigned".

If the status is currently "Open, unassigned" or is changed to "Open, 
unassigned", then the value of the field should be set to <blank> (and it would 
not appear on the Info Box, per paragraph 1).

Note: This requires completion of Issue 107 first.
