Change the information shown in the dialog on /dev/map

When a user clicks on a site on the map, they get a dialog with information 
about that site. We need to change the information on dialog box that pops up. 
For now, let's have it say:

Case Number: (This is just "A" + database ID, which is a number)
Name: Requestor name
Work requested:
Status:

Also please add a "Claim button", and a drop down to change the status, but 
don't hook them up yet (we'll use XHR here).
