If Work Order is Claimed, Gray "Claim for [Org Name]" on Assessment Form

You're not supposed to be able to claim a work order that has already been 
claimed.
At least gray out "Claim for [Org Name]" on the work order form if it has 
already been claimed.

Alternatively, we could change it to a link that says "Request from [Org which 
has already claimed it]". That link would open up a dialog box containing the 
contact information for the organization that has claimed it.  The person could 
then contact the organization that had claimed it.
