Date of request is automatically set already, for when the data is entered in
the form.

Note: Date of request should be editable, because some requests are written on
paper and may not be entered until several days later.

HTML Title should be "Collaborative Disaster Recovery Work Order System: Enter
a Work Order."
"Requestor name" should be renamed "Resident Name".
Under "Flood-Related Damage, Floors affected:" the default should be "--Choose
One--", as not all work orders will be floods. The value of "--Choose One--"
should be "".
In "Primary work type" the default key should be "--Choose One--", as not all
work orders are going to be floods. The value of "--Choose One--" should be
"". Also, add "Goods & Services" as another key, with the value "Goods". The
key "Trees" should be "Trees or Wind", with a value of "Trees".
"Number of trees wider than 18 inches" default should be 0.

Change "Requesting Only Debris Removal" to "Only Outside Debris Removal"
Add "Hardwood Floor Removal []" under "Carpet Removal"

Also leaving this out for now:
"Note: Status must be "Open, Unassigned" or "Closed, Duplicate" if
Organization="None." If organization is changed, user is prompted to enter
organization password (authentication). If status is set to Closed, Completed,
Incomplete, Out of Scope or Done by Others, prompt user to enter: 1. Number of
volunteers 2. Number of hours, and 3. Initials of resident present during work."
