Upon Closure, prompt for More Info #62

Upon changing the work order status to "Closed," prompt users for:

((# volunteers and # hours) or # volunteer-hours) and Initials of person 
present during work.

See "Validation" issue. Zero is an acceptable number.  Initials are optional.
