Work Order Input Form Changes

Changes to the UI:
* Change the width of the Work Order Input Form from ~600px to ~450px. Some 
people have very small screens.
* Change "Rent/Own" to "Rent/Own/Public" and add "Public Land" and "Business" 
to the drop-down menu.
* Move "Zip Code" below "State"
* Change "Member of assessing organization" to "Member of Your Organization"
* Change "Primary work type" to "*Primary Damage Type"
* Add "Food" to "Primary work type" drop-down.  Use attached icons for "Food" 
type.
* Change "Number of Tarps Needed" to "Number of 20' x 30' Tarps Needed"
* Change "Requests for Goods and Services" to "Goods, Services, or Food 
Requested"
* Change "Downed Wires" to "Downed Wires on Property"
* Change title, "Check-Out" to "Claim and Status"
* Change "Check-Out to [Org Name]:" to "Claim for [Org Name]:"
* Below, "Initials of resident present during work," add a new field, "Status 
Notes." This field is to allow the volunteer to add explanation about the 
status. E.g. "This person has two boats and a porche." or "Told us to come back 
later."

UPDATE:
* Change title/heading, "Check-Out" to "Claim, Status and Report"
* Add date field "Date Closed" below "Current Status". This field automatically 
fills in when status is changed from "Open..." to "Closed..."
* Delete heading "Completion Report"

One last change before pushing out from the sandbox:  Under the "Other 
Information" heading, please add a non-editable "Reported by: [Org Name]", 
above "Prepared by: [   ]"
