Constructor of ContainmentIndex is not backwards-compatible with 1.1.0 #92

The signature of the ContainmentIndex has changed as part of fixing issue 58. 

Users are not supposed to call the constructor, but instead to use the create() 
methods. The constructor will be made private in future releases.
