'Add another Tool parameter' duplicates first parameter

Hi,

Have noticed that when creating or configuring a tool, that hitting the bottom 
'+ Add another tool parameter' link will add an new empty parameter at the 
bottom, but also make a duplicate of the first parameter at the start of the 
list.

Seemt to be reproducable, just open any tool (or setup a new one with at least 
one parameter) and hit  '+ Add another tool parameter'

Seen in firefox and ie.

This is a minor issue.

Also renumbers the tool parameters

eg. at first it #0, #1, #2

then when you click

its #1, #2, #3, #4

with the old #0 in both the #1 and #2 slot.

This is probably a Mango issue... investigating...

May be worth leaving this one until mango1.3 is installed. This problem may go 
away then.
