Comments and formatting of push sample JS files

• Adding some more doc comments to try to pull it all together a little better.
• Formatting (mostly indent stuff) to make the code easier to read.
• Brought app name, org name, notifier name, and sender ID up into global variables so it's easier for users to insert values and run.
