Remove the automatically generated "base" and "user" layer. #436

Make it possible that datasets have no annotation layers. When people want to 
add annotations, force them to create, and specify a layer first. 

The current setup with base layer is too non-specific. I would change all the 
populated "base" layers in the current database to "Seizure" layers if I could.
