Add latitude and longitude form options #24

Information

Currnet system only accepts US zip codes/cities/states
It is possible to use the application if you have lat and lon but nothing else
Forms don't have this as a submission option

Deliverables

Add lat and lon form entries
Create logic around receiving lat and lon and bypassing geocode api.
