README should indicate the relation with the built-in exporter #3

It is not clear from the readme how the metrics differ from Patroni's built-in /metrics API endpoint (and whether those metrics are incorporated as well (they are not)). (There seems to be lot of overlap, but the format used differ)

Including that info would be useful.
