Problem with ampersands (&) in file names

I uploaded a PDF file with an ampersand in the name and I noticed that I could not download it. Needs further investigation

the same;

function fix_filename() converts & to & which is probably intended
then the name gets encoded several times turning $amp; into sequence of several amps .. don't know why; maybe htmlencode() function is wrong
