Create follow-up documentation #48

Right now the construction and export of follow-up items are a bit intricate. The construction of follow-up items must be fed with items (the text that describes the use, characteristics, etc. of the follow-up test) and questions (e.g. should the person undergo the follow-up?). To format to html and qualtrics, build and to export follow-up items it is necessary to create the items and question using placeholders. The documentation should make explicit the necessary placeholders and the use of them.
