In the original article (Beck, Epstein, Brown, & Steer, 1988), the BAI has no exact instructions nor labels on the middle points of its Likert Scale.
"The respondent is asked to rate how much he or she has been bothered by each symptom over the past week on a 4-point scale ranging from 0 (Not at all) to 3 (Severely—I could barely stand it). The items are summed to obtain a total score that can range from 0 to 63."

0 - Not at all
1 - Midly, but it didn't bother me much
2 - Moderately - it wasn't pleasent at times
3 - Severely - it bothered me a lot
