Clean up follow-up or future visits in encounter table #24

  1. Look at all follow-up/ future visits in encounter table - can they be part of CDM Encounter table based on CDM specifications?
  2. At MU, encounter table had followup visits for dead patients. These are not real encounters since they did not happen.
