Add module name macros #25

It could be nice if NGX_SSL_CT, NGX_HTTP_SSL_CT, NGX_MAIL_SSL_CT, and NGX_STREAM_SSL_CT are defined? They can be used elsewhere to detect whether a module is enabled.
