Distribution Entry: UI for Subcategory Entry #211

Instead of a "Subcategory" box, there should be a "Manual Entry" link that turns the dropdown box into a text field, so that you can specify a custom category name. (clicking "Manual Entry" is then replaced with "Pre-configured Entry" which brings back the dropdown when clicked.)

See my other comment. We shouldn't distinguish between between top level categories and lower level categories.
