PGs should be unitalicized by default even if in the middle of an italicized string, e.g.:
~A la recherche (ROH-sharesh) du temps perdu~
should come out with the PG formatted bold and unitalicized as normal, in the middle of the italicized title.
You could add functionality to manually override this and italicize them, but I can't imagine why anyone would ever need to do that. Much more common are situations like the above where allowing the guide to be written without manually ending and restarting the italics on the title would be convenient, and possibly prevent a big error if the question is edited without noticing the unusual tildes.
