Creating a PR template for issue #114. Very open to feedback on what to include/change. Looking to keep it relatively simple and lightweight to start. Can always add more later. I think that looks good for a start. Only thing that immediately comes to mind is that not every change will have screenshots applicable (refactors, cleanup, certain bug fixes, etc.), so that should probably be optional

@jasoncavanaugh, made some changes to be clear screenshots are needed only for UI changes
