Because we're looking at taking more patches from folks who are not Gravitational employees,
this patch adds some contribution guidelines to planet. Changes include:
Introduce a CODE_OF_CONDUCT and CONTRIBUTING guidelines (brought over
from Gravity)
Consolidate docs/README.md into the root README.md
Switch from footnote style links to inline (which makes them more likely to stay current).
