Enhancement Request for "Setup Responses" process

When someone is setting up responses by default PWM pre-fills "some" of the 
questions that we as admistrators have setup possibly indicating to the users 
(even though there is a drop box) that those are the questions we want them to 
answer.  However we have more questions (7 total I think) for the user to 
choose from.  We were wondering if it was possible to change it so that the 
drop-down was pre-filled with some verbiage like "Please click the arrow to 
select a question" or something like that (coding this so that this option is 
not selectable of course).  I am thinking that this would tell the user that 
they can select from a list of questions rather than thinking that "these are 
the questions".

Is this possible?
