Forgotten Password Unable to Find Username

What steps will reproduce the problem?
1. Once I have entered challenge questions for a user and can login as that 
user successfully, I then cannot use the forgot my password feature. 
2. I answer the challenge questions successfully, but it gives me a PWM 5016 
error. 
3.

What is the expected output? What do you see instead?
I expect to see either the current password or a way to recover the password. 
Do you have to have the email settings configured?

What version of PWM are you using?
1.6.4

What ldap directory and version are you using?
AD 2008 R2

Please paste any error log messages below:
Unable to find username. Please try again. { 5016 ERROR_CANT_MATCH_USER (error 
during contextless login username search, setting 'LDAP Directory-LDAP 
Contextless Login Root' value does not appear to be correct: Unprocessed 
Continuation Reference(s)) }

This functionality has been updated in recent builds.  Please try again with a 
nightly build and re-open this issue if you still have the error.
