user cannot change his challenges

What steps will reproduce the problem?
1. I`m not able to reporoduce this issue for other users.

What is the expected output? What do you see instead?
From time to time a user cannot change his challenge reponse set. When the user 
try to change his challenges, he get a PWM error 5022 and when click on the 
continue button, user is logged out. 
It is strange, because the response set was set up and the attributes like 
pwmResponseSet exists in the eDirectory. I see it via iManager or ConsoleOne. 
And when I deleted this attribute, I get the same error. When I delete a 
response set attribute for other user, I`m able to set new challenges. Also I`m 
not sure if the problem is in the eDirectory or in the PWM portal.

What version of PWM are you using?
1.6.1

What ldap directory and version are you using?
eDir - novell-NDSbase-8.8.6.3-15

oh, sorry. My customer turned off a password policy for some users and he 
forgot on this. So, PWM 1.6.1 is still working without problems.
