Images not loaded in sample since 2.0.8 #59

While debugging I found that our samples are not anymore sync with the release 
of XSLT stylesheets, it causes the images to not be loaded (at least in 
entityref)

DTD definitions in samples must be removed in order to properly use the XSL-NS 
stylesheets.

done in r218

samples do not use anymore xml dtd definitions, except the one defined for this 
purpose inside 4.4/ directory
