nccn guideline submission does not allow cutaneous melanoma #1301

Trying to submit the cutaneous melanoma guidelines (e.g. for AID 7) is not a valid value. The list of guideline documents by disease site should constitute allowable entries for this field.

I believe https://github.com/griffithlab/civic-client/issues/1255 addresses that. You should be able to add a new nccn guideline in the admin interface
