Update Evidence Level Text in the ADD Evidence Item form #1331

Minor issue regarding updating the text that appears below the Evidence Level on the form. Currently: A Proven/consensus association in human medicine B Clinical trial or other primary patient data supports association C Individual case reports from clinical journals D In vivo or in vitro models support association E Indirect evidence

These could be expanded to give a bit more thorough explanation and be fully inline with SOP. This came up on a ClinGen Somatic Heme WG call.
