assertions need to display an additional disclaimer #1543

On or around the main assertion view/summary, we need to include a disclaimer in addition to the homepage & footer disclaimers. At minimum, it should state "For Research Use Only", but might be extended. The shortened disclaimer should either display a popover when clicked, or link to a disclaimer page in the docs containing extended disclaimer text, something like:

"Disclaimer: The information in this variant assertion is not intended for direct medical decision-making without consulting a physician or other qualified health provider. Individuals should not change their health behavior solely on the basis of information contained on in this variant assertion. If you have questions about the information contained on this website, please see a health care professional."
