option for no age/birthday #9

some food banks record ages, some record birthdates, some record neither even if they record either of these, many do not require an entry

need an option in settings as to whether you want to collect ages or birthdays and whether they are required. also should not display fields for them if the options are off

How about adding a 'Food Bank Settings' option on the tab bar? Could be an option to check which fields you want to collect on Intake - by default would just be the core fields (not including BDay or Age).
