Making too many messages/notifications could also cause this.

In general, spam. If you look at non-moderated comment sites, you almost always see lots of spam. People put comments like, "I'm a single mom working from home and made $32,314 last month! See more at http://make-more-money-today-123-easy.com". For qa we sorta have this covered, for bids sorta since it's not public, although it might annoy users, especially for comments and messages it can be a problem, since anyone logged in can comment and send a private message. So in general we need an anti-spam idea. Hacker News doesn't allow you to submit more than once an hour or so, that's kinda extreme, doesn't work with private messaging. There's pre-approval of users, whereby you submit to join and we have to manually approve. There's moderation of all comments, this doesn't prevent spam to individuals but reduces public comment spam.
