Provide a user-interface that displays the structure of the DAISY book and allow users to navigate using the structural elements.

DAISY books inherently include a structure, even for simple books with only 1 
level of navigation. DAISY 2.02 supports up to 6 levels. 

HCH suggests we add a user-interface that presents users with the structure, 
and that allows users navigate directly to the relevant section of the book.

The DaisyReader already has a basic feature like this available but the feature 
was removed many months back because it was crude and not easy to find. So, the 
aim is to revive the earlier feature and make it easy to find and use.
