Parameterise certificate ARN #41

This moves the certificate ARN from hardcoded in the CFN to being a parameter.

I've applied this to the CODE and PROD load balancers.

Note that there was an apt-get upgrade step that was in the repo but not applied to the CF stack. This is also now applied.
