UI bug: trying to import with missing prefix means that the user has to "Choose File" again as well #212

What steps will reproduce the problem?
1. http://vocab.deri.ie/neologism/import
2. Choose file but do NOT indicate a namespace
3. "Namespace prefix field is required."
4. User adds prefix and clicks "Import from File"
5. Get "You must select a file for upload."  ****This is the problem -- my file 
choice has been forgotten****

What is the expected output? What do you see instead?

Expect that adding a prefix and clicking "Import from File" would fix it. 
(Problem is step #5)

Instead:
My file choice has been forgotten. In effect, along with the error message 
"Namespace prefix field is required.", the file is "unchosen". So in addition 
to adding a prefix,  the file needs to be reattached with "Choose File" before 
clicking  "Import from File".

Please provide any additional information below.
This is a severe UI bug since it causes a cascade of error events for the user.
