6 - hide 'insert into post' button on photo upload lightbox #6

What steps will reproduce the problem?
1. Upload photo using wpss.
2. Two buttons appear, 'insert photo' and 'insert photo in post'

What is the expected output? What do you see instead?

There should just be the one button, insert photo.

Should be fixed, tested on ff 3.5
