Adding custom text field values in html file and calling them in mod_alert_helper file and pushing them to jira under custom fields #196

So basically for this page, I want to add new html text boxes and take those text values and push it to JIRA from mod_alert file to the JIRA, but it never works and im not sure where its breaking,

and updating code here, any help on this would be really helpful, this has been such a blocker for me,
