Missing titles in `/v2/titles`

Current Behavior

Currently quite a few titles are missing in the API. Some of them used to be available, some of them I am not sure. These are the most reported ones (but there are plenty more):

Cod Swimming Amongst Mere Minnows
Community Warlord
Gazed into the Void
The Great Equalizer

Expected Behavior

The missing titles are available.

Steps To Reproduce

Go to https://api.guildwars2.com/v2/titles?ids=all
CTRL + F for "Cod Swimming"

A bit early to say this for sure, but I'll write this now before I forget. The new title introduced with the Black Lion Patron Pack is also missing.
