duplicate numbers appearing #10

All the numbers in my address book card appear twice, once in the format
1324567890 and once as (123) 456 7890.

Forgot to add that the fax number is also appearing ... probably should be 
excluded.

I'm seeing this also, once potentially auto loaded from the address book, and 
once for the numbers that were 
used to initiate calls (I think)
