Make a component to handle other fields in drop down menu

Make the following a separate component and use it whenever an "Other" Option is in a dropdown.

The component should make a text box appear whenever "Other" is selected. Whatever is entered into this textbox should be saved into the dropdown field in the backdown as "Other: "
