RFC: Use GitHub issue forms for new service ticket workflows

Description of the RFC

Using GitHub issue forms instead of issue description templates help Hack Clubbers to submit access requests to Hack Club infra with custom web form fields and can be extracted via https://github.com/marketplace/actions/issueops-form-parser.

I am creating this issue to collect feedback about this before I do send a merge request/patch containing these new issue forms based off new issue links in #hq-engineering Slack channel link bookmarks.
