Added content to Personal Information of the Intake Form.

1. I talked to Marie about the Today’s date field and we agreed that instead of adding a button for Inserting Current Date this field should be of type: date so users will have a calendar there to choose the date.
2. I made some changes to: Form/shared/BasicField.js and Form/shared/RadioButton.js to make them more generic and to fit my cases.
3. At this point I just added most of the needed content but didn't finish it all the way, I understand that someone is going to re-organize the Form components anyway.
4. In Income source we still need to add a text field for other.
5. We still need to add ‘Does this person have secondary name, email, or phone number? At the end of Contact Information.
