~@doctorsandy Would you~ please work on the consent form for participants under 18 issue template? The issue to create the template should ask what the lowest age of the participant type is and if it's under 18, guide them through creating a formal consent form.
We have the consent documents from previous years (https://github.com/hackforla/internship/wiki/Intern-Research-Plans-and-Goals#consent), but we need to make them into templates and provide step by step instructions so other UXRs can customize the documents and google forms for future use.
