Expenditure entry: Make address1, city, state, zip optional

This PR makes the address for reporting expenditures optional (gh-1128).

DB updates need to occur as well:

• [x] connect directly to db and DROP NOT NULL for address1, state, city, zip
• [ ] re-deploy
