Adapt CCTS NDR Exporter to current version of the NDR #92

Current state:

The exporter has been implemented against "Version 3.0,
Draft for the 2nd Public Review of ODP5, 30 December 2008".

The current version is "Version 3.0, Implementation Verification, Second
Iteration, 30 July 2009".

What to do:

Adapt the Exporter to the new version.

Tasks:

For each of the exporters (BDT, BIE, DOC), do the following:
 * Write tests that cover the parts of the specification to be implemented.
 * Document which parts of the spec are not implemented.
 * Implement the tests.
