update exam instructions (no use of full names)

I am grading student exams fully anonymously. However, some students use their first and last names in the file names that they submit.

Please update the exam instructions on

• [ ] oDCM, and
• [ ] dPrep

so that it is crystal clear that students may not indicate their full names ever on submitted documents via TestVision.

That way, I can be sure I can grade exams anonymously.
